Clearances

ALL EMPLOYEES OF THE WINDBER AREA SCHOOL DISTRICT MUST HAVE THE FOLLOWING UP-TO-DATE CLEARANCES BEFORE WORKING WITH OUR STUDENTS. CLEARANCES MUST BE UPDATED EVERY FIVE YEARS ONCE EMPLOYED BY OUR DISTRICT. ALL COACHES AND VOLUNTEERS MUST ALSO OBTAIN CLEARANCES AND UPDATE THEM EVERY FIVE YEARS.

All employees of Windber Area School District must provide the following documentation prior to the first day of employment:

  1. State Police Clearance (Act 34)

  2. Child Abuse Clearance (Act 151)

  3. FBI Fingerprints (Act 114): The School District Service Code # is 1KG6XN

  4. School Health Record: Please provide a School Health Record, including a physical and TB test, conducted within the past year.

  5. Act 24 (PDE Form 6004) - Arrest/Conviction Report

  6. PA Sexual Misconduct/Abuse Disclosure Release.  A separate form must be completed for:

    • Current employer
    • All former employers that were school entities
    • All former employers where you were in a position that involved direct contact with children.
  7. Act 126 Mandated Reporter Training
